Neal et June discutent dans un parc où la petite fille de June, Samantha joue au football. Neal fait la remarque qu’elle n’a pas l’air malade mais June inquiète lui révèle que la petite a besoin d’un nouveau rein et qu’elle a été retirée de la liste des greffes la semaine passée. Neal propose alors son aide.

Un peu plus tard Neal interrompt Peter et Elizabeth en plein petit déjeuner pour mettre Peter au courant de la situation de Samantha. Peter est intéressé quand celui-ci apprend qu’une femme d’une organisation a approché June peu de temps après que Samantha ait été retirée de la liste pour lui proposer de lui trouver un rein en échange d’une donation de 100 000 dollars. Il demande donc à Neal de trouver plus d’informations.

Neal en compagnie de Mozzie se prépare pour rencontrer Melissa Calloway qui représente la fameuse association. Neal a l’intention de se faire passer pour le conseiller financier de June afin de trouver plus d’informations permettant à Peter d’ouvrir une affaire. Observant Melissa arriver ils remarquent qu’elle laisse son attaché case dans la voiture et Neal demande à Mozzie d’aller le récupérer. Celui-ci refusant d’agir en plein jour, Neal décide de s’en charger et demande à Mozzie de prendre sa place auprès de June. Mozzie inquiet lui demande des conseils et Neal lui répond simplement de faire ce que lui ferait.

Dès que Neal tente d’ouvrir la voiture un policier arrive. Caffrey lui fait croire qu’il a enfermé son attaché case dans sa voiture avec tous ses papiers. Il ajoute qu’il est assistant du procureur et qu’il doit se rendre au tribunal pour une affaire où un policier s’est fait tirer dessus. Le policier gobe son histoire et demande à l’un des ses collègues de lui ouvrir la voiture.

Pendant ce temps un Mozzie très nerveux converse avec Calloway et June tout en observant les faits de Neal grâce à une caméra de surveillance. Melissa leur explique que l’organisation trouve des organes grâce à de bons samaritains et que l’argent qu’il demande permet de faire vivre l’association. Elle ajoute qu’ils doivent prendre leur décision rapidement. Après quoi elle s’apprête à partir. Mozzie qui tente de gagner du temps pour Neal qui est toujours dans la voiture invite Melissa à dîner mais celle-ci refuse. Une fois la femme sortie Mozzie appelle Neal qui dans la voiture prend des photos de ce qu’il trouve dans l’attaché case de Melissa. Il ne peut pas finir mais trouve avant de sortir une invitation pour un tournois de tennis privé, au West Side Tennis Club de Forest Hills.

Au bureau, Peter explique qu’il a cherché des informations sur l’association de Melissa, Hearts Wide Open. Celle-ci a été fondée par un homme très respectable le Dr Wayne Powell. Neal n’est pas de cet avis et dit à Peter qu’il a rencontré Calloway, ce qui ne plaît pas à l’agent. Neal ajoute qu’il a trouvé des noms. C’est à ce moment là que Jones les interrompt pour donner les résultats de ses recherches sur les fameux noms de Neal. Ils correspondent tous à des personnes travaillant pour la Doctoral Global Initiative. Les deux hommes décident finalement de rencontrer Powell et Neal suggère de profiter du tournoi de tennis.

Ils se rendent donc à l’évènement de charité où on ne les laisse pas entrer n’ayant pas d’invitation. Neal décide de prendre les choses en mains lorsqu’il voit Melissa arriver. Il lui dit être membre de Doctoral Global Initiative et être le Dr Parker. Il lui explique avoir oublié son invitation mais elle ne marche pas pour autant. Ce n’est que lorsqu’il lui présente Peter qui se fait  passer pour un chiropracteur, le Dr Tannenbaum, qu’elle les laisse entrer, visiblement attirée Burke.

Pendant que Peter offre un verre à Melissa, Neal va à l’encontre du Dr Powell auprès de qui il se fait donc passer pour un médecin qui a des contacts en Inde. Rapidement Powell essaye de recruter Neal et lui parle d’un patient qui cherche un rein qu’il pourrait peut être trouver en Inde. Il ajoute qu’il sera se montrer très généreux si c’était le cas. Peter se voit pour sa part obliger de démontrer ses talents de chiropracteur à Melissa qui lui laisse sa carte et son numéro et lui propose de lui faire visiter la clinique où travaille Powell.

De retour au bureau, Peter informe Neal de ses dernières découvertes sur Powell. Il voyage beaucoup en Inde et il n’a pas trouvé trace d’illégalité dans ses actions de charité. Il a néanmoins découvert qu’il souffrait de troubles rénaux qui demandant une greffe rénale. Les deux hommes pensent donc que l’organisation servirait en fait à lui trouver ce fameux rein. Peter pense qu’il pourrait trouver des preuves sur le lieu de travail de Powell, la Howser Clinic, mais la confidentialité médecin/patient les empêche d’y aller.

Plus tard ce soir là Neal apprend de Mozzie que l’organisation a retiré sa proposition pour la petite fille de June. Ils auraient rencontré un cas plus urgent. Mais Mozzie qui a surveillé la clinique a découvert que les employés se sont débarrassés d’un grand nombre de dossiers. Malheureusement il n’a pas pu y avoir accès.

Chez Peter, Elizabeth qui range le linge tombe sur la carte de Melissa dans l’une des poches de la veste de Peter et demande donc des explications à son mari. Peter gêné lui explique qu’il a du flirter avec elle pour que Neal puisse parler avec Powell. Contrairement à ce qu’il imaginait la réaction d’Elizabeth est très différente de ce qu’il attendait. En effet El ne peut s’empêcher de rire, à l’idée de son mari en train de flirter. Ils sont interrompus par un appel de Neal qui veut savoir si le FBI a commencé son enquête sur la clinique, ce qui est confirmé par Peter. Le bureau a demandé les dossiers financiers de l’établissement. Après avoir raccroché Neal explique la situation à Moz, et ils comprennent qu’ils sont la raison pour laquelle l’association ne veut plus s’occuper de Samantha. Ils décident donc de se rendre eux même à la clinique.

Le lendemain, Mozzie se fait passer pour un patient très confus et Neal pour son médecin qui l’adresse à la clinique Howser. Neal parvient ainsi à entrer dans le bureau du Dr Powell et trouve une liste de donneurs. Malheureusement la sécurité ne tarde pas à arriver, Neal arrive néanmoins à faxer à Peter une partie de la liste avant d’être maîtrisé et drogué sur l’ordre de Powell.

Après avoir reçu le fax de Neal, Peter utilise le système GPS du bracelet électronique et découvre que Neal est à la clinique. Enervé il explique à Elizabeth qu’il a besoin d’un mandat pour entrer dans la clinique et que Neal risque de retourner en prison. Elizabeth lui propose alors d’utiliser Melissa pour entrer dans la clinique, si elle l’invite il n’aura pas besoin de  mandat. Cette solution plaît à Peter qui appelle immédiatement Melissa, grâce à l’aide de sa femme Peter se voit inviter à la clinique le jour même. Malheureusement pour lui Melissa évoque « ses mains magiques » un détail qu’il avait omit de préciser à Elizabeth…

A la clinique Peter rencontre Melissa, qui est surprise de le voir arrivé si tôt. Elle tente de le faire revenir plus tard mais l’agent profite d’un appel de Powell pour monter discrètement dans les bureaux. Dans les couloirs il entend Neal qui sous l’effet de la drogue chante… Alors qu’il veut le détacher Neal lui montre que c’est déjà fait mais étant en train de planer il ne peut tenir debout seul, Peter le traine ainsi jusqu’à une salle de conférence. Neal sait qu’il va devoir retourner en prison et finit par avouer à Peter qu’il est la seule et unique personne en qui il a confiance. Peter menotte Neal à une chaise et lui demande de l’attendre, quand il revient il a la cassette de la vidéo surveillance, évitant ainsi le retour en prison de Neal.

De retour chez Peter, Elizabeth s’occupe de celui-ci et ne semble pas avoir pardonné à Peter ses cachoteries. Neal qui se sent un peu mieux dit à Peter qu’il a trouvé une liste de riches clients prêts à payer pour des organes et une liste avec des centaines de noms et de groupes sanguins. Cette dernière est celle qu’il a faxée à Peter. Seulement 4 noms sont arrivés à destinations mais Peter pense que cela sera suffisant.

Lors d’une réunion au bureau Peter explique ce qu’il a trouvé grâce aux quatre noms obtenus par Neal.  Powell ne trouve pas ses donneurs à l’étranger mais grâce aux cas de Hearts Wide Open. Il peut aussi expliquer le lien de Powell avec Doctoral Global Initiative. Powell qui a besoin d’un nouveau rein est à la recherche d’un donneur et utilise l’argent de l’œuvre de charité pour financer ses recherches. Malheureusement aucun des donneurs sur la liste ne veut témoigner. Ils doivent donc trouver une autre solution pour faire tomber Powell. Neal suggère de lui faire utiliser l’argent de l’arnaque pour ce qu’il veut le plus, un nouveau rein.

Ils mettent donc en place un plan pour faire croire à Powell que le rein qu’il lui reste est en train de lâcher. Sachant que Powell laisse ses costumes au pressing ils échangent l’un d’entre eux avec un plus grand pour lui faire croire qu’il a perdu du poids. Ils utilisent aussi un spray sur le costume afin de provoquer un rash chez Powell. Enfin grâce à Mozzie ils arrivent à ajouter du colorant alimentaire dans les bouteilles de cranberry qu’il se fait livrer afin de colorer ses urines pour qu’il pense perdre du sang.

Leur plan est un succès. Neal contacte donc Powell en se faisant passer pour le Dr Parker de DGI. Il lui dit avoir trouvé un rein en Inde et qu’il devrait venir immédiatement. Se faisant passer pour son chauffeur Jones, propose de l’eau à Powell qui prend à son habitudes des somnifères avant un vol.

Quand Powell se réveille, il est dans un lit d’hôpital. Neal vient le voir pour lui dire que son rein a lâché pendant le vol et qu’ils ont du lui retirer ce qui explique la dialyse. Powell demande pour le donneur mais Neal lui réclame 30 millions de dollars. D’abord Powell dit ne pas avoir une telle somme, mais Neal lui fait comprendre qu’il ne négociera pas et que s’il veut ce rein il doit payer. Finalement Powell lui parle d’un compte sécurisé où il laisse l’argent de l’œuvre de charité. Il donne le numéro du compte et le mot de passe à Neal qui  quitte la pièce. La dialyse s’arrête alors de fonctionner, Powell paniqué découvre alors la supercherie et quitte le bâtiment pour se retrouver dans les rues de New York où Peter et le reste de l’équipe se font un plaisir de l’arrêter.

De retour au parc, Peter et Neal rendent visite à June pour lui dire qu’ils ont eu Powell et son organisation. Peter a d’autres bonnes nouvelles puisqu’il lui apprend que Samantha est de nouveau sur la liste. Peter laisse June et Neal pour aller se faire pardonner après l’incident des « mains magiques ».
